Online Care enables care coordination by providing the infrastructure for primary care physicians to assemble a multi-disciplinary team of care providers online, to collaborate and manage a common care plan on a patient’s behalf. The team is able to communicate with one another, immediately and as frequently as needed, helping to close many of the communication gaps that exist today.
