Online Care Team Edition

American Well Online Care Team Edition, commercially available from Q1, 2010 allows providers to deliver coordinated care by collaborating with one another on behalf of a patient. Team Edition is offered to health plans as a stand-alone product or as part of the Online Care Suite.

Provider-to-Provider

Provider-to-provider tools allow the primary care physician and his or her patient to reach out and consult with another specialist in real-time, during an actual office visit. All the physician needs to do is log on, search for available specialists (e.g. allergists, cardiologists) and initiate an Online Care conversation with the specialist immediately.

How It Works:

FOR THE PCP
 
STEP 1: FIND A SPECIALIST
PCP realizes during the patient visit that she needs to see a specialist. PCP and her patient they search for an available specialist (e.g. allergist).
 
STEP 2: CONSULT A SPECIALIST
PCP and specialist discuss the patients' symptoms. PCP performs physical examination guided by the specialist.
 
STEP 3: PORT YOUR INFO
PCP prescribes as needed. Patient goes home without a need for a referral. FOR THE CONSULTING SPECIALIST
 
STEP 1: GO ONLINE
Physician advises the system of availability via Web.
 
STEP 2: CONNECT WITH PCP
Physician reviews an incoming request for a consultation and upon acceptance begins the interaction with the PCP.
 
STEP 3: WRAP UP CONVERSATION
Consulting physician annotates the consultation summary and gets paid.

Medical Home

The Online Medical Home allows primary care physicians to establish medical home teams on behalf of their patients and coordinate the collaboration needed to advance their care. When an Online Medical Home is created for a patient, all participating clinicians can come together to review patient health information, read transcripts of interactions with other providers, and receive alerts about gaps in care from powerful clinical analytics. They can also discuss the patient’s health and care plan, either through the team’s discussion board or in real-time conversations using video, chat or telephone.
